Read-only consent example. This page does not collect a phone number, verify ownership, or record consent. It describes the intended enrollment flow for invited reviewers. Live SMS and cross-demo synchronization are not yet enabled.
What reviewers are joining
This optional program demonstrates notification workflows in Code Forged Systems software prototypes. Messages relate only to demos a reviewer requests or agrees to evaluate: a mobile verification code, a requested demo notification, or a demo workflow update.
No marketing, production client traffic, real patient information, financial information, or other sensitive data is sent through the program. The intended audience is no more than five invited, operator-approved reviewers, with fewer than 100 outbound SMS segments across the program each month.
Enrollment disclosure
The live consent box must start unchecked. Mobile verification and consent must be recorded as separate events.
Public example only. Enrollment is for invited reviewers through a verified demo flow.
Intended enrollment workflow
- An invited reviewer signs in to a Code Forged Systems demo portal.
- The reviewer adds a mobile number and requests a six-digit verification code, with a disclosure explaining the requested verification message.
- The reviewer reads the unchecked consent disclosure, checks the box, enters the code, and chooses to verify and join.
- The application records mobile-number verification and explicit consent separately, including UTC time, source, and the disclosure version.
- Demo SMS is permitted only when the contact is verified, consent is active, the reviewer is approved, and neither a program-wide nor product-specific suppression applies.
Changing your mind
Reply STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, or QUIT to stop program SMS. The intended re-enrollment flow is START followed by YES. Reply HELP for help, or email dpoulson@codeforgedsystems.com. Other clear opt-out requests can be made through the coordinator.
In-app demo notifications remain available after SMS opt-out. EMR-specific preferences continue to be controlled inside the EMR.
Implementation status
This public example is not proof that live enrollment has been completed. Before it is submitted for carrier review as evidence of an operating flow, the live portal wording and behavior must match it and the actual enrollment process must be documented. Existing EMR disclosures have not been changed by this website.
